Permission Forms
Permission Forms must be filled out for all events for everyone attending -- puppyraisers, parents, additional family members and guests. Please give the filled out permission form to the club member collecting forms for the event. Everyone attending must fill out one of these two forms:

Adult slip -- http://njaes.rutgers.edu/pubs/publication.asp?pid=4H103
In addition, if you are a youth puppyraiser attending an vent without your parent, this form must also be filled out and you must ask another adult club member to be responsible for you at that event.
